Assertion Failure in squid 2.6STABLE21

Hi there,

I am getting repeated assertion failures while using an NTLM
authentication helper:

2010/01/07 10:32:45| AuthenticateNTLMHandleReply: Helper '0x8c8c250' crashed!.
2010/01/07 10:32:45| assertion failed: helper.c:332: "!srv->request"

Any help or advise on how to trouble-shoot this will be much appreciated.
