Ahh. This is the same server that failed earlier today (for which the
above debug was generated) - it hasnt been restarted yet and still has
no traffic going through it (apart from our monitoring systems
occasionally generating test requests).

This looks like it confirms my above thoughts where these requests are
somehow getting stuck.
