Squid-3 NT

Hi,

After a long "fight" with Microsoft C++ "features", I'm finally able to
build Squid-3 NT using Visual Studio.

On the MinGW side this is the current building configure options:
./configure --disable-wccp --enable-underscores --enable-htcp
--enable-cpu-profiling \
  --enable-default-hostsfile=none --enable-removal-policies="heap lru" \
  --enable-auth="basic ntlm digest"
--enable-basic-auth-helpers="win32_locallogon NCSA" \
  --enable-err-languages="English Italian"
--enable-default-err-language=English \
  --enable-storeio="ufs awin32 null" --enable-referer-log
--enable-useragent-log \
  --enable-external-acl-helpers="win32_group ip_user"
--enable-ntlm-auth-helpers="NTLMSSP-WIN32 fakeauth" \
  --enable-win32-service --enable-snmp --prefix=c:/MinGWsquid --enable-icmp
--enable-ssl

conversion.
