The decoding/encoding of the three authentication NTLMSSP structures
is not a problem. The problem the "NTLMSSP" helper is dealing with is
the mapping between NTLMSSP and SMBsessionSetupX using SMB
authentication (not SMB encapsulated NTLMSSP).

Note: The name NTLMSSP is quite inappropiate for this Squid NTLM auth
helper. It is not more of an NTLMSSP auth helper than the other NTLM
auth helpers. All NTLM auth helpers implement NTLMSSP between the
helper and the browser, none to the auth server.
